- comments: observed approximately 75 small rodent droppings in the following areas during time of inspection: serving area behind 2 door reach in cooler, inside floor drains located in main prep, under prep table near bulk containers, in dish room scattered on countertop/ under heavy equipment, in dry storage under shevling units and 1st floor electrical room outside cafeteria entry. Instructed to clean and sanitize said area must eliminate pest activity. 7-38-020 (a). No citation issued.

- comments: during inspection, found 2nd floor boy's toilet room 1 out 5 urinals were inoperable and covered with garbage bag; instructed to repair and maintain in good condition

- comments: observed during inspection walk-in cooler with food debris and black-like substance on floor, instructed to clean and maintain said area.
