- comments: person in charge unable to provide a signed employee health policy for each employee. Instructed to provide. Priority foundation violation 7-38-010.
5. Procedures for responding to vomiting and diarrheal events
- comments: person in charge unable to provide vomit kit and procedure to follow when responding to vomit and diarrheal events. Instructed to provide vomit kit with the necessary items. Priority foundation violation 7-38-005.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand washing signs or posters near hand washing sinks in food preparation area and inside washrooms. Must provide.
38. Insects, rodents, & animals not present
- comments: observed two live roaches crawling on floor underneath 2-door prep cooler. Instructed to clean and sanitized affected area. Additional pest control service is needed to eliminate roach activity. Priority foundation violation 7-38-020. Citation issued.
40. Personal cleanliness
- comments: observed employee not wearing hair restraint in food preparation area. Must wear hair restraints at all times food is being prepared and served.
41. Wiping cloths: properly used & stored
- comments: observed wiping cloths stored on top of prep tables. Must properly store wiping cloths in a sanitizong solution when not in use.

- comments: previous core violations not corrected from inspection report#2286184 on 05/02/2019. #37 must label food storage containers when food is not in original package. #55 must repair or replace damaged floor tiles in prep area and toiletrooms,loose wall bases in dining area. #55 must elevate,remove and organize clutter in rear porch area,rear yard and throughout basement. #56 must provide ventilation in men's toilet room. #58 observed no food allergen certificate posted to view for food manager. 61. Summary report displayed and visible to the public
- comments: observed no previous inspection report summary posted on the premises in view of the public. Management was given a copy of today's inspection report summary and was instructed to post it in view of all customers. Priority foundation 7-42-010(b)
