- comments: observed cold tcs food not maintaining proper temperature chicken 56. 2f, cheese 53. 5f, sausage 49. 8f. Instructed manager to discard all cold food must maintain 41f or below during preparation, storage, display and service. Priorty 7-38-005 citation issued

- comments: observed coolers in pizza prep area not maintaining proper temperature, prep cooler 60. 1f, reach-in cooler 53. 3f. Instructed manager to repair and maintain coolers at 41f or below. Coolers are tagged held for inspection and must not be used until reinspected by chicago department of health. Priority 7-38-005 citation issued

- comments: observed over 80 rat droppings scattered on floor along walls and corners in prep, dishwashing and storage areas. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed food handlers not wearing hair restraints. All food handlers must wear effective hair restraints.

- comments: observed toilet not flushing properly in washroom in rear prep area. Instructed manager to repair and maintain.
