- comments: observed hand washing sink with hose tied up to it. Instructed manager to remove hose and ensure the hand washing sink is for hand washing only. Manager removed hose. Priority foundation violation 7-38-030(c) no citation issued.

- comments: observed mozzarella cheese sticks held at an inadequate temperature of 47 f. Instructed manager to ensure all cold tcs foods are held at an adequate temperature of 41 f and below. Manager discarded 4 pounds of mozzarella cheese sticks ($15) priority violation 7-38-005 no citation issued.

- comments: observed no hot water in right sink in girl's toilet room on 2nd floor. Hot water in right sink was measured at 100 f. Instructed manager to provide hot water with a minimum temperature of 100 f to all hand washing sinks

Report #2546571 dated january 18th 2022 instructed facility to repaint grease trap underneath 3 compartment sink with excessive rust accumulation by april 18th 2022. Observed grease trap with excessive rust accumulation. Instructed manager to paint grease trap. Priority foundation violation 7-42-090 no citation issued.
