2. City of chicago food service sanitation certificate
- comments: observed facility food handlers preparing and handling time and temperature control for safety(tcs) foods milk, vegetables, and turkey) without original city of chicago food service manager and certificate on site. Informed person in charge of the city of chicago original food service certificate must remain posted on site and certified manager on site during tcs food prep, cooking, serving, and operating hours. Priority foundation violation #7-38-012. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no paper towel available at handsink in in-use dishwashing room during time of inspection. Instructed to provide and maintain at all times proper handwashing while washing, rinsing, and sanitizing kitchenware and utensils. Priority foundation violation. #7-38-030 (c). Citation issued. Manager provided at this time.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed ice machine located in prep area internal lid with a pink slimy build up on internal panel dripping onto ice used for human consumption. Instructed to clean food contact surfaces and maintain. Priority foundation violation. #7-38-005. Citation issued.

36. Thermometers provided & accurate
- comments: observed no thermometers inside the cooling units located in the prep area. Instructed to provide to maintain internal temperatures.

38. Insects, rodents, & animals not present
- comments: observed approximately 85 small live black flies on the walls and exterior surfaces of heavy equipment, flying throughout storage in the rear prep area where kitchenware is stored, and flying around 3 compartment sink/ice machine. Noted approximately 66 small rodent droppings in the following areas. . . In the dishwashing room, underneath heavy equipment (dishwasher and drying racks), scattered on the floor in 1st floor dry storage room behind a large cabinet, under bulk containers with wheels and on the floor, behind the walk-in freezer, and inside the pump room on the the floor in corners. Instructed to contact pest control for additional service. Instructed to remove all droppings, clean and sanitize, and eliminate live fly activity. Priority foundation violation. 7-38-020 (a). Citation issued.

49. Non-food/food contact surfaces clean
- comments: observed rolling white bulk containers and shelving units with accumulated mold built on non-food contact surfaces stored inside of walk-in cooler and inside 2-door cooling units stored inside prep area. Instructed to clean at frequency and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed spilled food and dried debris on floors throughout prep area. Instructed to remove to prevent pest harborage.

56. Adequate ventilation & lighting; designated areas used
- comments: observed mechanical ventilation inside employee toilet room (behind red door) 1srt floor inoperable. Instructed to repair and maintain in good condition.

57. All food employees have food handler training
- comments: observed food handlers without food handler training. Instructed to obtain.
