- comments: upon arrival, observed no certified food manager on site while employees were preparing food items and for the entire walk through out the facility. - comments: observed black mold like substance on the interior of the panel above the ice insisde ice machine on site. Instructed to clean, sanitize and maintain. Ice machine is used for human consumption. Priority foundation violation 7-38-005, citation issued

- comments: observed small and bulk food storage containers not labeled in the rear food prep area. Instructed to label all food storage containers with food common name.

- comments: observed the rear exit door not rodent proof at the bottom. Instructed to rodent proof mentioned door and maintain.

- comments: observed a bin of clear containers of powder on the floor in the rear food prep area. Instructed to refrain from storing food items on the floor and maintain.

- comments: observed food handlers in prep areas without hair restraints. Instructed all food handlers must wear hair restraints while in rear food prep area.

- comments: observed clutter in the hot water tank closet in the rear food prep area. Instructed to declutter and maintain mentioned area.
