2. City of chicago food service sanitation certificate
- comments: no original verifiable city of chicago sanitation certificate on site at time of inspection. Instructed person in charge to provide original certificate as well as certified manager at all times. Priority foundation 7-38-012. Citation issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed insect particles on interior wall of ice machine (ice used for human consumption). Instructed person in charge to remove and discard of all ice, clean and sanitize machine and maintain. Priority foundation violation #7-38-005. Citation issued
35. Approved thawing methods used
- comments: observed meat being thawed in large bowl on floor of basement. Instructed person in charge to thaw with running potable water in clean and sanitized container in prep sink.
38. Insects, rodents, & animals not present
- comments: observed facility to be infested with approximately 100 multi-generational roaches in the following areas:in upstairs prep area underneath all prep tables, under three compartment sink near dish machine, on hot holding cabinet, on walls. As well as basement prep area in the following areas: inside ice machine, on walls behind sinks, on garbage containers, in gaskets of freezer units, underneath prep tables and in containers holding utensils. Instructed person in charge to obtain additonal pest control service to mitigate activity, clean and sanitize all affected areas, and maintain at all times. Priority violation 7-38-020(a). Citation issued.
38. Insects, rodents, & animals not present
- comments: observed approximately 150 rat droppings throughout upstairs and downstairs prep and dry storage areas in the following areas: in corners of beverage prep area, under three compartment sink, in metal shelf near sink. Instructed person in charge to remove all droppings, sanitize all areas, and inform pest control of need for additional service. Priority foundation #7-38-020(a). Citation issued.
44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: observed dishware and cook ware improperly stored, not inverted. Instructed person in charge to invert all dishware and cookware to prevent contamination.
49. Non-food/food contact surfaces clean
- comments: observed excessive build up of grease and food debris on all cooking equipment in upstairs and basement prep area, on the side of tables, wok stations, and stoves. Instructed person in charge to detail clean and maintain.

- comments: observed walls, tables, floor, and all sinks to be dirty with residue and covered with grease. Instructed person in charge to detail clean and maintain.
