- comments: observed no vomit and diarrhea clean up kit and procedure on site. Instructed to provide all necessary supplies as outlined in policy. Priority foundation. 7-38-005.

- comments: observed no employee hand washing signage at hand sink in prep area. Must provide.
38. Insects, rodents, & animals not present
- comments: observed pest control log book is incomplete. One report was provided for the 2020 calendar year. Last service was done on september 8, 2020. Instructed manager a licensed exterminator must conduct insect and vermin services at the establishment at least twice a year. Priority foundation. 7-38-020(c).

- comments: observed dirty cutting board in kitchen prep area with dark stains and deep grooves. Must detail clean and maintain.
