- comments: 5-203. 11-- must provide a handwashing sink with hot and cold running water, soap and paper towels in the dry food storage/prep room where two prep tables are located and foods being prepared during the inspection. Or remove all prep tables and equipment.

- comments: the menu does not not disclose or inform the consumers of potential hazards of consuming raw or undercooked animal origin foods such as burger. Must provide a consumer advisory that discloses and reminds consumers. Priority foundation violation.
