1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed person in charge without proof of food safety management training during time of inspection. Must provide.

2. City of chicago food service sanitation certificate
- comments: noted facility did not provide a valid city of chicago food service sanitation certificate for employees working on site. Informed a certified manager with a current city of chicago certificate must be on premises at all times and a valid must remain posted on site whenever time and temperature control for safety(tcs) foods are being prepared, handled and/or served. Priority foundation violation 7-38-012. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed hand sink in basement prep area not accessible. Basin was filled with hose and strainer. Instructed that sink must be accessible and maintained at all times. Priority foundation violation 7-38-030(c). Citation issued.

- comments: observed no paper towel at the hand-sink in the prep area. Informed person in charge that paper towels must be available at hand sinks at all times. Food handler obtained paper towel for hand sink during time of inspection priority foundation violation 7-38-030(c). Citation combined with above violation

16. Food-contact surfaces: cleaned & sanitized
- comments: observed a white panel inside the ice machine that touched ice filled with a black mold-like substance. Instructed manager to wash, clean, sanitize and maintain. Ice is used for human consumption. Priority foundation violation. 7-38-005. Citation issued.

28. Toxic substances properly identified, stored, & used
- comments: observed open rodent bait (rodenticide) in food prep and storage area. Observed rodent bait (rodenticide) on food storage shelves and around condiments located in basement areas. Instructed to remove, clean, sanitize and maintain all rodenticide in bait boxes. Priority violation. 7-38-015 citation issued.

38. Insects, rodents, & animals not present
- comments: rodent infestation through-out the premises. Observed approximately 150 rat droppings throughout facility on both levels: on the floor of the basement food prep/dishwash area, in rear stairwell leading to basement door & rear patio bar. In office/hot water tank & liquor storage area, on shelving where equipment is stored, behind ice machine and under the storage shelves, in hvac closet and main bar floor near fireplace. Also noted additional pest activity with approx. 45 live small flies noted during inspection. Small fly activity was noted at bar and basement food prep. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority 7-38-020(a). Citation issued.

- comments: observed a 1/4" gap at the bottom of the front entrance door. Must provide a tight-fitting seal at all outer openings to prevent the entry of pests.

- comments: observed missing ice bin covers at front bar and patio bar ice bins. Must provide and maintain ice bin covers.

- comments: observed ice scoops being stored in ice bins. Instructed on how to properly store ice scoop between uses.

44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: observed pot and pans and not inverted. Instructed to invert utensils after sanitizing and air drying.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed cutting boards in the rear prep area with open seams and dark stains. Instructed to refinish or replace damaged or stained cutting boards and maintain at all times.

- comments: observed raw wood surfaces, made to hold dump sinks at bar front counter area. Instructed to seal or paint to provide a smooth and easily cleanable surface.

- comments: observed the interior of the walk in cooler walls and fan cover guards in need of cleaning. Instructed to detail clean.

- comments: observed grease buildup, food debris, litter and dirt on floor under cooking equipment, along walls and in all corners in bar, prep areas, dish washing, storage and basement areas. Instructed manager to clean floors under all equipment, along walls and in all corners in all areas. Observed dust buildup on light shields in cooking area and ceiling and wall vents in prep and storage areas. Instructed manager to clean and maintain all fans, light shields and ceiling/wall vents.

- comments: observed chipped paint and holes in the ceiling of basement near dish and over the soda syrup boxes. Instructed to smoothly seal.

- comments: observed no proof of training for all food handlers. Instructed to provide and maintain records at all times.

- comments: observed no certified food manager with allergen training. Instructed any persons with the city of chicago food sanitation certificate to comply with completion of training. Must provide documentation.
