- comments: observed timed faucets not functioning properly at hand washing sinks in washrooms. Water must run contnuously for at least 15 seconds before reactivating faucet.

- comments: observed over 100 mice droppings scattered on floor in unused dish room and canteen, on electical outlets and boxes in kitchen, on floor along walls and corners of compressor room in kitchen, on floor under shelving in dry storage area, on floor along walls in walk-in freezer and ice machine storage area, on floor along walls in dock area behind folding tables, milk crates and pipes, on floor along file cabinet men kitchen staff washroom, on floor along walls and corners near vending machine in teacher lounge 122, on floor along walls and shelving in storage room and ledges behind copiers in second floor teachers lounge, on floor along walls and corners in basement electrical rooms. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a)

- comments: observed 1/4' gap on bottom left corner of exit #3 near dock and bottom right corner exit #1 at main entrance. Instructed manager to seal all outer openings on doors to prevent pest entry.

- comments: observed dust buildup on interior and exterior surfaces of cooking equipment, shelving units, prep tables, holding units in prep area. Instructed manager to clean and maintain.

- comments: observed no hot water at right hand washing sinks in second floor boys washroom near room 223 and left sink in women gym washroom. Instructed manager to repair.

- comments: observed no covered waste receptacle in women gym washroom. Instructed manager to provide.

- comments: observed openings in walls, around pipe fittings, along baseboards in prep, storage, and office areas. Instucted manager to seal all openings in walls and ceilings, along baseboards, in all areas.
