2. City of chicago food service sanitation certificate
- comments: observed no certified food manager present with a valid city of chicago food sanitation certificate while tcs foods (rice at 135f) were being prepared. Management instructed that a certified food manager must be present at all times while tcs foods are being prepared or served. Priority foundation violation 7-38-012. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed approx. 10 live small flying insects behind the bar and on the walls, in the dish washing area and basement food prep area. Also observed approx. 40 live ants on the basement food prep table and storage shelves. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a). Citation issued.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed broken plastic food storage containers in the basement. Observed single use aluminum foil containers being reused. Management instructed that single use aluminum foil containers cannot be washed and reused.

49. Non-food/food contact surfaces clean
- comments: clean the basement food prep tables and food storage containers. Clean the door tracks to the 2 door prep cooler in the basement.

57. All food employees have food handler training
- comments: observed the food handling employee without a food handler certificate. Management instructed that all food handling employees must show proof of training.

58. Allergen training as required
- comments: observed no valid allergen training certificates on the premises. Management instructed that all certified food managers must show valid proof of training.
