- comments: observed no certified manager on duty while tcs items such as chicken were prepared from 1:40-2:30 pm. The certified manager arrived on site at 2:30 pm. Instructed to maintain employee holding valid city of chicago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012. Citation issued.

- comments: observed rear door does not lock. Instructed to repair and maintain tight fitting outer openings.

- comments: observed build up of dirt and food debris on can opener in rear dish/prep area. Instructed to detail clean and maintain in good condition.

- comments: observed heavy build up of food debris, dirt, grease, and grime on floor underneath cook line and prep cooler. Instructed to detail clean and maintain in good condition.
