- comments: observed no written employee health policy provided on the premises. Management instructed to have a signed employee health policy for each employee. Priority foundation violation 7-38-010. Citation issued.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean up supplies on the premises to respond to a vomit or diarrheal event. Management instructed to provide. Priority foundation violation 7-38-005. Citation issued.
15. Food separated and protected
- comments: observed cross contamination of cooked foods and raw vegetables during storage in the walk-in cooler. Observed cooked chicken, crab rangoon and raw cut vegetables (carrot, green onions and peppers) stored in boxes that were previously used to store raw chicken from the supplier. Management instructed not to store any food in boxes that were previously used to store raw chicken. Priority violation 7-38-005. Citation issued.

- comments: observed cooked eggrolls stored inside of a cardboard box. Management instructed to store all food in proper food storage containers.

- comments: observed torn door gaskets inside of the doors to the 2 door prep cooler. Management instructed to replace the torn door gaskets.

- comments: clean the following items in the food prep area: prep tables and storage shelves, excessive grease around and on top of the wok station, smoker unit, bulk food storage containers, storage shelves in the walk-in cooler, and deep fryers exterior.

- comments: no covered waste receptacle was observed in the employee washroom. Management instructed to provide.
56. Adequate ventilation & lighting; designated areas used
- comments: clean the grease buildup on the ventilation hood and filters.
57. All food employees have food handler training
- comments: observed a food handling employee with an expired food handler certificate. Management instructed to show valid proof of training for all employees.
58. Allergen training as required
- comments: observed the certified food manager with an expired allergen training certificate. Must show proof of valid certificate.
60. Previous core violation corrected
- comments: previous core violations from report #2500578 on 4/22/21 not corrected: 51 - prep handwash bowl faucet leaking. Must repair and maintain sink. 55 - observed some water stained ceiling tiles above prep area. Must replace damaged ceiling tiles. Priority foundation violation 7-42-090. Citation issued.

- comments: no inspection report summary was posted on the premises. Management was given a copy of today's inspection report summary and was instructed to post it in view of all customers. Priority foundation violation 7-42-010(b). Citation issued.
