- comments: observed over 100 mice droppings scattered on floor under 3-compartment sink, along walls and corners in kitchen, on floor along walls and corners inside infant room behind regrigerator and cubbies, on shelving units in classroom, on floor along walls and corners in basement furnace and utility rooms. Observed live roach crawling on table and live roach crawling on wall in lobby. Instructed manager to call an exterminator for service, clean and santizie all areas. Priority foundation 7-38-020(a) citation issued.

- comments: observed sinks, refrigerator, counter, holding units, prep tables, cubbies, shelving units in need of cleaning. Instructed manager to clean and maintain.

- comments: observed no covered waste receptacle inside staff washroom. Instructed manager to provide and maintain.

Instructed manager to seal all openings in walls and ceilings in all areas.

- comments: observed unused articles, tire, etc inside infant room. Instructed manager to remove all unnecessary items to prevent pest harborage.

- comments: previous core violations not corrected from report #2582062 on 9-20-23:47-found the washbowls in the children's toilet room in need of recaulking. Instructed manager to have sinks recaulked and maintain. 51-found no back flow prevetion device on the rearmop sink. Instructed manager to instal back flow prevention device to mop sink and maintain. 55-found water stained ceiling tiles in the classrooms. Instructedmanager to replace all water stained ceiling tiles and maintain. Priority foundation 7-42-090 citation issued
