- comments: observed no handwashing signs at handsinks. Instructed person in charge to provide and maintain.

- comments: observed dead mouse behind large cold hold unit on south wall. Instructed person in charge to remove dead rodent and sanitize area.

56. Adequate ventilation & lighting; designated areas used
- comments: observed build up of grease on filters in hood ventilation system over cooking equipment.

60. Previous core violation corrected
- comments: observed previous core violations from report: #2555198 dated: 5/3/22 not corrected; #51 observed no back flow device on the rear utility sink. Instructed to provide and maintain in good working order. #51 observed cold knob missing (water turns on) repair knob and maintain in good working order. #55 observed several open floor areas throughout the facility. Instructed to seal and maintain all areas as a means of making the floors easy to clean at all times. #55 observed a hole in the ceiling tile in the front area where the lottery/security station will be. Instructed to repair and mainatin. Also observed a ceiling tile in the rear prep/dish washing area with a stained tile over area. Instructed to replace and maintain. Priority foundation 7-42-090. Citation issued.
