- comments: observed no employee health policy on site as required. Priority foundation violatioan 7-38-010. Instructed the person in charge to provide documentation of employees responsibility to report any illness transmissible through food. Citation issued.

- comments: observed no handwashing signage at the handwashing sink located near the three compartment sink in the front bar area. Instructed to provide handwashing signage informing employees to wash their hands and maintain.
37. Food properly labeled; original container
- comments: observed bulk food containers stored in wooden cabinets in pizza preparation area with no labels. Instructed to label bulk food containers when food is not in original package.
38. Insects, rodents, & animals not present
- comments: observed a dead mouse on glue trap in the basment. Instructed to remove dead mouse from premises and maintain.

- comments: observed plastic grocery bags being used to store food items in freezer located in basement. Instructed to use food grade bags or containers to store food items.

- comments: observed no chemical test kit that accurately measures the concentration in mg/l of sanitizing solution on site. Priority foundation violatioan 7-38-005. Instructed the person in charge to provide a chemical test kit or other device that accurately measures the concentration in mg/l of sanitizing solutions and maintain. Citation issued.

- comments: observed dirty floors with food debris/ spills throughout kitchen preparation area. Instructed manager to detail clean and maintain
