- comments: observed no hand soap available at the following hands during the time of inspection. . . (in use) front prep area, (in use) rear prep/dishwashing area, and employee unisex toilet room. Instructed to provide and maintain at all times while open and operating. Priority foundation violation. #7-38-030 (c). Citation issued.

- comments: observed handsink in (in use) rear prep/dishwashing area without hot water during time of inspection. Noted handsink temperature @50f. Instructed to provide hot water at required temperature of at least 100f. Priority violation. 7-38-030 (c) citation. Issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed (in use) the low-temperature dish machine in the rear prep area not properly sanitized during the time of inspection. Noted sanitizer strength @ 0ppm. Instructed person in charge to discontinue use of machine until machine is properly sanitized as required by code, must maintain while open and operating priority violation. #7-38-025. Citation issued.

- comments: observed evidence of pest activity on site. Noted approximately 8 large rodent droppings on floor and shelving unit in bar attached to dining area. Instructed to remove, clean and sanitize. Instructed to eliminate pesat activity on premise. Priority foundation violation. #7-38-020 (a). Citation issued.

- comments: observed all cutting boards on site with dark stains and deep cuts. Instructed to replace.
55. Physical facilities installed, maintained & clean
- comments: observed broken/missing floor tiles in front prep area near exit door throughout main prep area where grill and all heavy equipment is located, under all shelving units, middle storage space where cooling/freezer units are stored, rear prep/dishwashing area as well as in large rear prep in back of facility. Instructed to replace tiles, remove standing puddles to prevent pest harborage, and provide smooth and easily cleanable.

- comments: observed rear prep/dishwashing ceiling tiles falling from ceiling in during time of inspection. Instructed to replace all missing and water damaged ceiling tiles in said area and mintain facility.

- comments: observdd excessive clutter and stacked items in front service area of dining area as well as bar. Instructed to declutter, remove unnecesary items, organize and elevate items at least 6 inches off floor in order to prevent pest harborage.
