- comments: observed no certified manager on site from 12:50-1:20 pm while tcs items such as chicken were prepared. Instructed to maintain an employee holding a valid city of chicago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no handwashing signage at exposed handwashing sink located at end of cook line. Instructed to provide.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed build-up of black slime in the interior of ice machine used to produce ice for human consumption. Instructed to remove ice, clean, sanitize, and maintain ice machine free of slime. Priority foundation violation 7-38-005. Citation issued.

22. Proper cold holding temperatures
- comments: observed the following tcs items held at the stated improper temperatures in 2 door cooler located in kitchen: crab rangoons between 48. 8 and 52. 7f, chicken at 49. 2f, and pork at 49. 8f. Management discarded approximately 140 lbs of product valued at approximately $800. Instructed to maintain all tcs items at or below 41f at all times. Priority violation 7-38-005. Citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed 2-door prep cooler containing tcs items such as chicken and pork maintaining an improper ambient temperature of 52. 5f. Cooler tagged and held for inspection. Instructed to repair or replace and maintain all coolers at or below 41f at all times. Priority violation 7-38-005. Citation issued.

- comments: observed dead roaches throughout facility. Instructed to remove and maintain facility free of dead pests.

39. Contamination prevented during food preparation, storage & display
- comments: observed solid aluminium foil used as liners on shelving and equipment. Instructed to remove and maintain smooth, cleanable surfaces.

43. In-use utensils: properly stored
- comments: observed tongs stored on handles to oven and fryers. Instructed to remove and store utensils properly.

- comments: observed mops stored mop head down. Instructed to store mops upside down to allow for proper drying.

- comments: observed torn gasket on cooler located in rear storage room. Instructed to repair or replace and maintain in good condition.
