- comments: 2-101. 11 observed no certified food manager on duty while tcs foods are being prepared, handled, such as (bacon, potatoes, salads, ceviches)must be on site at all times. Manager arrived on site at 12:00 p. M. (cos)(priority foundation 7-38-012)(citation issued)

- comments: 3-302. 12 : must label food storage containers when food is not in original package. (squeeze bottles)

- comments: 6-501. 111 observed evidence of appx. 40 or more live small flies on walls and flying around in front and rear bar areas, dining rooms, women's toilet room, and basement. Also observed standing clear water on basement floor on both sides. Must remove water from floor and keep dry. Recommended to have a pest control operator to service premises on or after 7/9/25.
