2. City of chicago food service sanitation certificate
- comments: observed facility without a valid city of chicago food service sanitation certificate posted onsite for food employees. Informed person in charge city of chicago original food service certificate must be posted along with the certified manager on site when time and temperature control for safety(tcs) foods (milk) are stored. Priority foundation violation #7-38-012. Citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees in acknowledgment of reporting agreement. Priority foundation violation #7-38-010. Citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no written procedures or supplies for responding to vomiting or diarrheal events. Supplies must include adequate sanitizer that eliminates norovirus. Instructed facility to provide clean-up policy and supplies to handle bodily fluid spills on site properly. Priority foundation violation #7-38-005. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed hand sink in the rear dish prep area was not accessible. Observed milk crates and empty boxes stored in front of the hand sink. Instructed that sink must be accessible and maintained at all times. Priority foundation violation #7-38-030(c). Citation issued.

13. Food in good condition, safe, & unadulterated
- comments: observed mold on containers of berries (rasberry & blackberry) not in good condition that protects the integrity of the contents, prevents exposure, or is free of potential contaminants stored in the rear 2-door refrigerator. The containers containing 2lbs worth $15 were discarded at the time of inspection to correct the violation. Priority foundation violation #7-38-005. Citation issued.

36. Thermometers provided & accurate
- comments: observed no metal stem thermometer on site to monitor the internal food temperature of tcs (milk, egg) food cooked, delivered and served. The facility must obtain and maintain proper thermometers. Priority foundation violation #7-38-005. Citation issued

38. Insects, rodents, & animals not present
- comments: observed no current pest control log book on the premises. Instructed the person in charge that a licensed pest control company and operator must provide at least 2 pest inspections a year, and the reports must be maintained in the log book. Instructed to provide a current pest control log book and pest inspection reports. Priority foundation 7-38-020(c). Citation issued.

40. Personal cleanliness
- comments: observed food handlers prepping food without hair restraints. Instructed the person in charge that all food handlers must wear a hair restraint.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed milk crates being used as shelving walk-in freezers in the prep and storage areas. Must remove and provide shelving 6' off the floor and provides floor accessibility to ensure adequate facility cleaning.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed facility without a quat chemical test kit for sanitizer or ware washing area. An informed person in charge of a test kit that accurately measures the concentration in mg/l of sanitizing solution must be provided. Priority foundation violation #7-38-005. Citation issued.

57. All food employees have food handler training
- comments: observed no proof of training for all food handlers. Instructed to provide and maintain records at all times.
