38. Insects, rodents, & animals not present
- comments: observed evidence of rodent activity on premises. Found 200 or more mouse droppings on the floor in the storage areas behind, under and around storage pallets, along wall and on shelf ledges, found droppings on the selling floor packaged food area on shelves, found droppings on the floor of the cooking area by the wok, inside the bottom of the fryer and on bottom shelf of prep tables, found droppings on the wall ledge in the small butcher room and on the floor by the talapia tank in the butcher area also observed in the boiler room on the floor, womens washroom on the floor by the handsink and in the storage room next to the employee break room, droppings observed in the area where disposal units are located inside of store on the floor and droppings observed on the floor and along wall ledge inside of two walk in units used to store dry (non refrigerated) products. Instructed to clean all areas and have pest control company service prior to reinspection next week. Priority foundation violation 7-38-020(a) issued for control of vermin and insects.

39. Contamination prevented during food preparation, storage & display
- comments: found no barrier at exposed handsink in the fish prep area at the exposed handsink; instructed to install a barrier/splash guard at side of sink as sink is next to a prep table where live fish are cleaned and gutted.

49. Non-food/food contact surfaces clean
- comments: instructed to clean the interior bottom of the fryer in the kitchen area to remove droppings and grease. Instructed to clean all storage shelves in the kitchen area and prep tables. Instructed to clean shelves inside of walk-in coolers where rusted. Instructed to clean shelves on selling floor where mouse droppings observed.

51. Plumbing installed; proper backflow devices
- comments: observed a missing tolet seat in the first stall of the mens washroom; instructed to replace toilet seat where needed.

55. Physical facilities installed, maintained & clean
- comments: instructed to clean the floor under around and behind all cooking equipment, disposal units, storage shelves on selling floor and storage areas. Instructed to clean the walls and ceilings inside of walk-in coolers where needed to remove mold like substance. Instructed to clean the fan covers inside of the walk in units to remove dust.

55. Physical facilities installed, maintained & clean
- comments: observed the wall coving behind one toilet in the in the mens washroom loose; instructed to reattach where needed.
