- comments: observed hand washing sink not accessible. Observed 2- wet towels and a small pan upside down inside of said sink. Articles were removed during inspection. Must keep accessible at all times. (priority foundation 7-38-030(c)(citation issued)

- comments: observed evidence of appx. 50 or more live small flies on packaged napkins,cable wires,ceilings in dish/prep area,bag of carrots on food storage shelf and flying around premises.
