- comments: observed no person-in-charge present with city of chicago certified food manager training or certificate on-site at time of inspection. Instructed to provide person-in-charge with certified food manager training and maintain certificate on-site at all times. Priority foundation violation.
2. City of chicago food service sanitation certificate
- comments: observed no food employee on-site with city of chicago certified food manager training or original certificate on-site while preparing tcs food items (sandwiches, pizza, salads). Instructed to provide at least one food employee with city of chicago certified food manager training and maintain original certificate(s) on-site and displayed in public view at all times of food operations. Priority foundation violation. 7-38-012 citation issued.
6. Proper eating, tasting, drinking, or tobacco use
- comments: observed employee drinking from open beverage container at the main cook line area. Instructed to provide designated areas for employees to eat, drink, or use tobacco that does not pose risk of contamination of food, clean equipment, or utensils and maintain all employee beverage containers enclosed.
39. Contamination prevented during food preparation, storage & display
- comments: observed food in original containers stored direcrtly on the floor in the walk-in cooler and dry storage areas. Observed several dry food storage containers (chili flakes, flour, sugar, seasoning) stored open without adequate overhead protection in the dry storage area. Instructed to maintain food storage containers at least 6 inches above the floor and adequately protected from overhead at all times.

- comments: see violation 60.
51. Plumbing installed; proper backflow devices
- comments: observed the following plumbing fixtures in poor repair: backflow prevention device at the utility sink missing cap, but functional; handwashing sink at the employee storage/walk-in cooler area in poor repair, faucet spraying when activated, but functional. Instructed to repair or replace the listed plumbing fixtures and maintain all plumbing fixtures in good repair.
57. All food employees have food handler training
- comments: observed food handling employees on-site without food handler training or certificates at time of inspection. Instructed to provide all food handling staff with approved food handler training and maintain certificate(s) on-site for inspector review at all times.
60. Previous core violation corrected
- comments: observed the following core violation from the previous complaint inspection not corrected at time of current complaint inspection: 48 4-301. 12 missing sink stoppers at 3-comp sink, instructed to provide. Instructed to replace missing sink stoppers at the 3-compartment sink and correct all core violations by the assigned correct by date. Priority foundation violation. 7-42-090 citation issued.
