- comments: observed no certified manager on site at time of inspection while tcs items such as chicken were prepared. Instructed to maintain an employee holding a valid city of chicago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no paper towels at exposed handwashing sink located in dish room. Instructed to maintain paper towels at all exposed handwashing sinks at all times. Priority foundation violation 7-38-030(c). Citation issued.

- comments: observed chicken thawing in bucket of water in walk-in cooler. Instructed to only thaw items under running water.
38. Insects, rodents, & animals not present
- comments: observed 5 rodent droppings on top of low temperature dish machine, approximately 20 rodent droppings behind prep cooler and walk-in cooler in rear prep area, and 15 flies throughout facility. Instructed to remove, receive additional pest control service, and maintain facility free of pest activity. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed ice scoop stored directly in ice chest at bar. Instructed to maintain ice scoop outside of ice to avoid contamination.

- comments: observed torn gaskets on prep coolers, drawer cooler under grill, and door of walk-in cooler. Instructed to repair or replace and maintain in good condition.

- comments: observed lid needed to garbage can inside the unisex (left) washroom. Instructed to provide.

- comments: observed heavy build up of grease, grime, and food debris on floors, walls, and equipment at bar, dish room, and food prep area. Instructed to detail clean and maintain in good condition.
