- comments: observed evidence of appx. 100 or more rat droppings on floor inside of 1st fl. Furnance room and scattered throughout basement along wallbases and shelving units. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 4/28/22. (priority foundation 7-38-020(a)(citation issued)

- comments: observed an appx. '1/2-3/4' gap along bottom of front and rear doors. Must make door tight fitting.

- comments: must eliminate crates used for food storage on sales floor. Provide adequate shelving units.

- comments: must repair or replace slow drain at hand sink in front prep area, missing sewer covers and large gaps at sewer covers in basement.

- comments: must remove excessive caulking around front window and wall next to 3- compartment sink to be smooth and easily cleanable.
