Report date: Dec 3, 20213. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed several employees who have not signed the employee health policy. Management instructed to have a signed employee health policy from each employee. Priority foundation 7-38-010.
38. Insects, rodents, & animals not present
- comments: observed evidence of rodent activity and live insect activity on the premises. Observed approximately 100 mouse droppings in the closet next to the exposed hand washing sink in the food prep area, on the ledge behind the storage rack and on the lower shelf of the prep table in the dish washing area, on the ledges of the elevated prep cooler, on the floor behind the chest freezers, and on the floor in the dry storage room and liquor storage room/office. Also observed approximately 30 live small flying insects around the bar area. Also observed one live cockroach on the wall next to the dish washing area. Additional pest control service is needed to eliminate all pest activity. Must clean all rodent droppings from all affected areas throughout. Priority foundation 7-38-020(a). Citation issued.

- comments: clean the prep tables in the food prep area, freezers and prep coolers, wire storage shelves above the 3 compartment sink, interior of deep fryers, and sinks, shelves, frozen margarita machine and liquor bottles behind the bar.

- comments: remove excessive clutter and unused equipment from the outside storage area behind the restaurant.

- comments: clean the grease on the floor underneath the cooking equipment. Clean the floor beneath the frozen margarita machine at the bar.

- comments: clean the excessive grease from the ventilation hood and filters above the cooking equipment.
57. All food employees have food handler training
- comments: observed several food handling employees without a food handler certificate. Must show proof of training for all employees.
58. Allergen training as required
- comments: observed no valid allergen training certificates for any of the certified food managers. Management instructed that all certified food managers must obtain the certificate.
60. Previous core violation corrected
- comments: previous core violation from report #2362611 on 2/20/20 not corrected: 53 - must provide toilet room receptacle with cover for sanitary napkins in women's washroom. Priority foundation 7-42-090.
