- comments: : 6-501. 114-- 6-201. 11-- floors throughout the entire facility, under all equipment, shelving, coolers with dirt and debris. Must clean and maintain. -----holes in walls throughout the rear dish and mop sink area. Must seal all holes/openings in walls throughout the facility.

- comments: 8-404. 13 (b:3) previous priority violation from 3-9-22 report #2552690 not corrected. #59- observed approximately over 50 mice droppings in the following areas: throughout the hvac storage closet, under the pizza oven, along baseboard behind cooks line equipment, along baseboards throughout the rear dry food storage room and rear prep area. In the washroom. Instructed to remove all droppings, sanitize all areas, and have facility serviced by licensed pest co. License remains suspended at this time by c. D. P. H.
