Access to the basement was not provided; a live mouse was seen in the rear storage room and mouse feces were found on shelving and floor perimeters; fruit flies were seen in food preparation area; a dead and live roach were found on food prep refrigerator surfaces; food employees were not wearing or were wearing ineffective hair restraints, such as nets, hats, or beard restraints; there were no single use gloves found; shelves throughout the establishment were found with debris and mouse feces; the ventilation unit inside toilet room was not working.
Due to imminent health hazards observed during this inspection, the establishment was issued a Cease Operations Order and must discontinue food operations immediately. The establishment could not operate for a minimum of 48 hours.
