- comments: observed improper hot holding temperatures of tcs food. Observed cooked rice at 126f inside of the hot holding unit. Management instructed to keep all hot foods above 135f. Priority violation 7-38-005. Citation issued.

22. Proper cold holding temperatures
- comments: observed improper cold holding temperatures of tcs foods inside of the prep coolers and in the prep area. Observed ceviche at 45f, shrimp at 45f, and octopus at 47f. Management instructed to maintain all cold foods below 41f. Priority violation 7-38-005. See violation #21 for citation.

38. Insects, rodents, & animals not present
- comments: observed no current pest control lob book from a licensed pest control company. Management instructed to have at least 2 pest inspection reports from the last year inside of the pest control log book and to maintain the log book on premises for review. Priority foundation violation 7-38-020(c). Citation issued.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: the women's washroom door lacks a self-closing door device. Management instructed to install.

57. All food employees have food handler training
- comments: observed a food handling employee without a food handler certificate. Management instructed that all food handling employees must show proof of training.

58. Allergen training as required
- comments: observed the certified food manager without a valid allergen training certificate. Management instructed that all certified food managers must show valid proof of training.
