- comments: observed mising labels on bulk containers. Instructed person in charge to label all bulk items not in original container with common name.

- comments: observed 3-compartment sink stoppers missing/not working at three compartment sink. Instructed manager must provide working stoppers at 3-compartment sink.

- comments: observed shelving in walk in cooler to be covered in rust and food debris. Instructed person in charge to replace/repair and maintain.

52. Sewage & waste water properly disposed
- comments: observed facility with inadequate wastewater disposal at drain in rear prep under heating equipment. Soiled water is observed coming from the drain onto the floor in the prep area when the handsink is used or hot hold equipment is emptied. The building plumber arrived on site to correct the plumbing. Water is not backing up at the floor drain at the time of the report. Priority violation 7-38-030(c). Citation issued.

- comments: observed no covered receptacle in women's restroom. Instructed person in charge to provide and maintain.

- comments: observed toilets and washbowls in men's and women's sinks to be soiled and men's toilets to be cracked at the base. Instructed the person in charge to clean all fixtures and repair or replace the toilet.

- comments: observed broken/missing wall tile in the middle prep area near the handsink and instructed the person in charge to repair all damaged wall tile and maintain it. Observed damaged floor tile in front fryers to be severely cracked. Instructed person in charge to repair and maintain.

- comments: observed vents in dining room, restrooms, and throughout prep areas to have accumulated dust. Instructed person in charge to clean and maintain.

- comments: observed previous core violations from report # 2587716, dated 1/8/24, not corrected; #49 observed build of grime on equipment throughout facility. Instructed person in charge to detail clean and maintain. #55-observed excessive build-up of food spills and grime on all walls and floor throughout the entire prep area. Instructed person in charge to detail clean and maintain. Priority foundation #7-42-090. Citation issued.
