- comments: observed no person with city of chicago food service manager training on site. Instructed person in charge that certified food service manager must be on site at all times.

- comments: observed manager on site without city of chicago food service manager certification. Instructed person in charge that certified food service manager must be on site, as well as original certificate at all times tcs foods (chicken, potatoes, cole slaw etc) are being prepared and served. Priority foundation violation 7-38-012 citation issued.

- comments: observed no paper towels at handsink in front prep area. Instructed person in charge that hand drying provision must be available at all times. Priority foundation 7-38-030 (c). Citation issued. Pic provided handdrying provision during inspection.

- comments: observed build up of food debris and residue on equipment and tables throughout rear prep area. Instructed person in charge to clean and maintain.

- comments: observed grease trap at three compartment sink to be leaking excessively and inoperable, causing waste water to drain onto food prep area floor. Instructed person in charge to repair and maintain at all times. Priority #7-38-030(c). Citation issued.
55. Physical facilities installed, maintained & clean
- comments: observed build up of food residue on floors throughout rear prep area. Instructed person in charge to clean and maintain.
