- comments: observed employee's filled beverage cup with no lid on bulk container under prep table. Instructed manager to provide closed lids on employee beverage containers to prevent contamination.

- comments: observed bulk containers not labeled under prep tables in prep area. Instructed manager to label all bulk containers with common name of food.
38. Insects, rodents, & animals not present
- comments: observed three live roaches crawling on floor and several roaches on glue boards in kitchen electrical and storage closets. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued.

- comments: observed final rinse gauge not operable on high temperature dish machine. Instructed manager to repair and maintain. Priority foundation 7-38-005 citation issued.
