- comments: found no procedure/plan for responding to vomiting and diarrheal events. Instructed to provide and maintain required procedure/plan and appropriate supplies or kit. (necessary items at a minimum: gloves, face masks, disposable mops and appropriate sanitizer/spray that kills norovirus) premises only has absorbent (saw dust) material available. Priority foundation violation 7-38-005 no citation issued

- comments: noted ice scoop stored on top of ice machine. Instructed to properly store ice scoop in a clean container to prevent contamination.

- comments: instructed to detail clean and maintain storage racks with dust or mold like accumulation inside walk-in cooler.

- comments: found no hot running water available throughout premises. Water temperature of 95. 0f. Must provide sufficient hot water temperature of 100. 0f for hand washing sinks and 110. 0f for 3 compartment sink. Instructed to fix. Priority violation 7-38-030 (c) no citation issued (issued 2 day notice)

- comments: found 3 compartment sink not properly maintained. Waste water backig-up from one compartment into another compartment of the 3 compartment sink. Instructed to fix. Priority violation 7-38-030 (c) no citation issued (issued 2 day notice)

- comments: instructed to detail clean and maintain ceiling vent cover with heavy dust build-up inside men and women's 1st floor (employee) washroom.

Yesterday afternoon purchased Food from Dunkin’ Donuts. It was a bacon egg and cheese croissant with an order of hash browns. Within 4/5 hours of consuming had started to vomit violently. Ate carne fries, and me and my boyfriend both became sick in the morning with stomach cramps and nonstop diarrhea 12 hours later. It was the only thing we ate in common. And there’s nothing else that could’ve given us this.
