- comments: observed approximately 30 small flies flying throughout the following areas during the time of inspection:. . . Fanduel bar, ultra club pantry, kettle one club bar (main kitchen and storage area). Must eliminate pest activity on premise and maintain. # 7-38-020 (a). Citation issued.

- comments: observed kitchenware pre-set on tables in kettle one club as well as ultra club bar uncovered. Instructed to provide protection against possible contamination.

- comments: observed raw wood wall in main prep area of queenies. Instructed to provide a sealed, smooth, and easily cleanable surface in said prep area.
