- comments: observed bulk containers not labeled in rear bakery. Instructed manager to label all bulk containers with common name of food.
38. Insects, rodents, & animals not present
- comments: observed over 100 mice droppings scattered on floor under shelving units, along walls and corners in single service and dry food storage areas. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued
39. Contamination prevented during food preparation, storage & display
- comments: observed food items stored on floor in prep, walk-in cooler and walk-in freezer. Instructed manager to elevate all food items six inches off floor.
41. Wiping cloths: properly used & stored
- comments: observed soiled wiping cloths stored on prep tables. Instructed manager to store wiping cloths in sanitizing solution between uses.
45. Single-use/single-service articles: properly stored & used
- comments: observed single service articles stored on floor in serving areas. Instructed manager to elevate all single service articles six inches off floor.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed rust deposits on shelving units in walk-in cooler, freezer, storage areas and prep tables in prep areas. Instructed manager to repair/replace and maintain.

- comments: observed accumulated grease buildup on interior and exterior surfaces of fryers, dust buildup on fan guards in walk-in coolers, food debris on prep tables and shelving units. Instructed manager yto clean and maintain.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacles in staff washrooms. Instructed manager to provide.
55. Physical facilities installed, maintained & clean
- comments: observed stained ceiling tiles in rear bakery prep area and front serving area, missing wall tiles in locker room, openings around electrical outlet near dairy cooler in sales area. Instructed manager to replace and maintain.
56. Adequate ventilation & lighting; designated areas used
- comments: observed ventilation fan not operable in washroom. Instructed manager to repair.
57. All food employees have food handler training
- comments: observed missing and expired food handler training. Instructed manager to provide valid food handler training for all employees.
