1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed person in charge without proof of food safety management training during time of inspection. Must provide.

2. City of chicago food service sanitation certificate
- comments: observed facility food handlers preparing and handling time and temperature control for safety(tcs) foods without original city of chicago food service manager and certificate on site. Informed person in charge city of chicago original food service certificate must remain posted on site and certified manager on site during tcs food prep, cooking, serving and operating hours. Manager arrived at end of walk thorugh. Priority foundation violation 7-38-012. Citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees in acknowledgment of reporting agreement. Priority foundation violation 7-38-010. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed hand sink in rear prep area not accessible. Basin was filled with metal food container. Instructed that sink must be accessible and maintained at all times. Pic removed items from hand sink. Priority foundation violation 7-38-030(c). Citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed the ice bin interior in need of cleaning. Instructed to remove ice, clean and maintain at all times.

- comments: observed the following food not meeting require temperature of 135f degrees or above in the designated hot-holding unit: scrambled eggs 114.0f. Manager immediately and voluntarily disposed of 2 lbs of products worth $15.00 through denaturing process. Priority violation 7-38-005. Citation issued.

- comments: noted internal temperatures of the following time/temperature control for safety food items inside the food display cooler to be improper: scrambled egg at 51.8f manager immediately and voluntarily disposed of 5 lbs of products worth $50.00 through denaturing process. Priority violation 7-38-005. Citation combined with above violation.

37. Food properly labeled; original container
- comments: observed packaged food items (cookies, scones, danishes, and croissants) not properly labeled. Instructed person in charge to properly label all packaged food items with identity, net weight, ingredients, business name and address where made and packaged.

38. Insects, rodents, & animals not present
- comments: observed a prohibited animal (dog) on site in the facility. Person in charge was made aware that such animals are prohibited from food establishment. Instructed to remove animal from the facility and maintain at all times. Dog was removed during inspection. Priority foundation 7-40-060. Citation issued.

- comments: noted fly traps hanging directly above prep table in prep area, dish and dry storage areas. According to food code: insect control devices shall be installed so that the devices are not located over a food preparation area. Must relocate existing fly trap devices in direct violation of this rule.

- comments: observed food containers stored on the floor in the storage areas. Instructed manager to properly store and protect food items from contamination while stored by elevating at least six inches from the floor on raised shelving units.

- comments: observed the interior of the reach in coolers in need of cleaning. Instructed to detail clean.

- comments: observed gaskets of all reach in cooler units encrusted in food debris instructed to clean and maintained at all times.

- comments: observed clutter stored on floor in hvac closet area. Instructed manager to remove all unused articles to prevent pest harborage.

- comments: observed no proof of training for all food handlers. Instructed to provide and maintain records at all times.

- comments: noted continued non-compliance from report 2616825 conducted on may 5, 2025. Previous core violations not corrected: (51) 5-205.15: **c** system maintained in good repair (in good repair) observed leaking faucet at the 3 compartment sink in the rear dish washing area. Sink faucet leaks from base when in use. Instructed to service and repair leak. 5-204.12: **c** backflow prevention device, location (no backflow device) observed ice machine at front prep area to be without a backflow prevention device. Instructed manager to provide/locate backflow prevention device for ice machine. Must correct core violations by the date on the report. Priority foundation violation 7-42-090. Citation issued.
