- comments: previous priority foundation violation from report #2578998 7/25/23 not corrected still observed evidence of 5- live roaches crawling on wheel bases of 2- door used & 2- door unused prep coolers and upper door trim in main prep area and 6- rat droppings on floor under & on side of booth seats in bar area along walls,on bricks of open wall in basement,on dollie underneath 1- door glass reach in cooler on rear porch storage area. Also, a dead mouse on trap in basement. Must remove dropping & dead mouse clean and sanitize areas. Must have a pest control operator to service premises on or after 8/1/23.
