- comments: observed no one on-site with a valid city of chicago certified food manager certificate. While time/ temperature controlled foods, including soups and sauces, are being cooked and cooled. Priority foundation violation 7-38-012

29. Compliance with variance/specialized process/haccp
- comments: observed a cook and chill process done on site,however no haccp plan provided on site nor to chicago department of public health. Food is cooked in kettles, food is transferred to bags which is sealed and stored in large compartment with ice and water. A record is kept of cooling process. Must provide haccp plan on site with requirement of the rules and submit to public health. Priority foundation violation 7-38-005 citation issued

55. Physical facilities installed, maintained & clean
- comments: observed ceiling above food storage and preparation area not smooth and easily cleanable. Must install non-absorbant drop ceiling over all areas where open food is prepared and stored.
