- comments: no original and valid city of chicago food service sanitation certificate posted nor provided on site for present manager on duty. Instructed to provide. A least one certified food manager must be on site when food is handled. Priority foundation: 7-38-012, citation issued.
38. Insects, rodents, & animals not present
- comments: we observed approx 50 rodent droppings under the bar area(west area of bar). Also rodent droppings at storage area. Instructed to remove,clean, sanitize and maintain. Priority foundation violation:7-38-020(a), citation issued

- comments: splash guard is needed between handwashing sink and shelf were clean utensils are stored in the kitchen. Surface must be smooth,cleanable and non-absorbent surface.
