- comments: observed no written procedures or supplies for responding to vomiting or diarrheal events. Supplies must include adequate sanitizer that eliminates norovirus. Instructed facility to provide clean up policy and supplies to properly handle bodily fluid spills on site. Priority foundation violation #7-38-005. No citation issued.

49. Non-food/food contact surfaces clean
- comments: observed an excessive amount of debris and ice buildup on the shelves/walls/ceiling of the one door freezer. Must remove excess ice, detail clean, and maintain.

51. Plumbing installed; proper backflow devices
- comments: observed the handsink in the toliet room to have a slow drain. Instructed to repair and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle in the employee toilet room. Instructed to provide.

59. Previous priority foundation violation corrected
- comments: observed previous priority foundation violation #38 (observed no current pest control log book on the premises. Instructed person in charge that a licensed pest control company and operator must provide at least 2 pest inspections a year and the reports must be maintained in the log book and pest inspection report. #49 (all raw wooden shelving throughout the entire establishment must be painted/sealed to be smooth and cleanable. ), #55 (water damaged and missing ceiling tiles throughout entire establishment must be replaced. -----peeling paint and chipping plaster on the walls and ceiling at the three compartment sink/prep area. Must remove/repair. -----all raw wooden beams, doorways, walls must be painted/sealed. -----noted expanding foam spray sealant along floor baseboards and on walls throughout establishment must be removed to provide smooth, cleanable floors/baseboards. -----all damaged/missing walls and ceiling at the rear storage room must be repaired to be smooth and cleanable)(all unused, unnecessary items stored throughout the facility must be removed and needed articles stored organized and elevated) and #57 (no proof of food handler training or certificate for food handler on site. Must provide) not corrected within specified time frame on the inspection report #2509348 dated for 05/07/21. Instructed person in charge that the violation shall now classified as a priority item due to noncompliance. 7-42-090 priority violation citation issued.
