5. Procedures for responding to vomiting and diarrheal events
- comments: observed no written procedures or supplies for responding to vomiting or diarrheal events. Supplies must include adequate sanitizer that eliminates norovirus. Instructed facility to provide clean-up policy and supplies to handle bodily fluid spills on site properly. Priority foundation violation #7-38-005. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed retail food establishments without a licensed pest control operator. All retail food establishments shall contract with a licensed illinois exterminator who shall provide insect and vermin services at the establishment at least twice a year. A log book shall be kept for twelve (12) months and include a copy of the service reports and the pest control company’s illinois department of public (idph) health license number. Instructed to provide. Priority foundation violation #7-38-020(c). Citation issued

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed 3-compartment sink stoppers missing/not working right, left, and middle compartments. Instructed manager must provide working stoppers at 3-compartment sink.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no quat chemical test kit available during inspection. Instructed manager to provide test kit for proper concentration in sanitizing solutions. Priority foundation 7-38-005 citation issued.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle in both unisex staff and public toilet rooms. Instructed to provide and maintain.
