35. Approved thawing methods used
- comments: observed two bins of frozen spinach sitting out on counter. Instructed to thaw in refrigerator that maintains a temperature of 41f or below or under running water at a temperature of 70f or below.

37. Food properly labeled; original container
- comments: observed bulk food containers that are not labeled throughout establishment (spices, rice). Instructed to provide the common name on each container that is out of its original packaging to properly identify food item.

39. Contamination prevented during food preparation, storage & display
- comments: observed food container boxes on floors in walk in cooler (vegetables and milk crates). Must elevate and store on clean shelves.

49. Non-food/food contact surfaces clean
- comments: observed heavy grease build up inside of cooking fryers at service prep area. Must clean and maintain.

51. Plumbing installed; proper backflow devices
- comments: observed minor leak at hand washing sink in main kitchen prep area in rear next to mop sink. Must repair. Observed water temperature at hand washing sink on right side outside of men's and women's bathrooms at 93. 2f. Proper water temperature must be at 100. 0f or above. There are two hand washing sinks located outside of men's and women's bathroom. Hand washing sink on left side was reaching proper water temperature of 100. 0f.

51. Plumbing installed; proper backflow devices
- comments: observed no visible back flow preventer device on ice machine water line. Must provide.

52. Sewage & waste water properly disposed
- comments: observed sewage backing up from floor drain under 3-compartment sink in service prep area and at 3-compartment sink at main kitchen prep area when water is being drained. Sewage must be completely drained into an approved sewage system and must be repaired and maintained at all times. Priority violation. 7-38-030(c). Citation issued.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no lid on trash can in women's and employee bathroom. Must provide.

55. Physical facilities installed, maintained & clean
- comments: observed dirty floors behind cooking equipment in service prep area with grease spills and food debris. Must detail clean and maintain.

- comments: observed allergen training certificates are incomplete. Instructed to provide allergen training for all employees who obtain the city of chicago food manager certificate.
