- comments: observed 45 small live ants crawling under cooling units, coffee storage across from donut disply (front prep) and rear prep/dish area behind cooking equipment and shelving racks crawling along baseboard and in grout spaces. Noted aproximatley 7 small rodent droppings in rear cake decorating area as well as 7 medium sized live black flies flying throughout prep areas. Intructed to contact pest control for service. Must eliminate pest activity on premise and maintain. Priority foundation violation. #7-38-012. Citation issued.

- comments: observed food handlers without effective hair restraint while preparing food. Instructed to provide and maitnain.

- comments: observed internal surfaces of donut display with excessive food debris built up. Instructed to clean and maintain in order to prevent pest harborage
