- comments: observed no chlorine chemical sanitizer on premises used to sanitize equipment and utensils. Corrected on site chlorine chemical sanitizer delivered to facility before conclusion of inspection. Instructed chlorine chemical sanitizer shall be provided and available for use during all hours of operation. Priorioty violation 7-38-005 citation issued.
38. Insects, rodents, & animals not present
- comments: observed two roaches in kitchen. Observed roach on prep table holding microwave and on interior of second compartment of three compartment sink. Corrected on site roaches killed and removed. Instructed to clean and sanitize affected areas. Priority foundation violation. 7-38-020(a) citation issued.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed unsealed wood to elevate two door oven in kitchen and hot holding unit. Instructed to smoothly seal wood in said areas for ease of cleanability.

- comments: observed buildup on exterior of small two door cold holding unit in kitchen. Instructed to detail clean exterior of small two door cold holding unit and maintain. Observed food buildup on interior base of both blenders. Instructed to clean and maintain. Observed food debris and dust on all shelving of prep table holding microwave in kitchen. Instructed to clean and maintain. Observed food debris on exterior of sandwich press. Instructed to clean and maintain. Observed debris on interior of two door cold holding unit used for dry storage. Instructed to clean and maintain.

- comments: observed no backflow prevention devices on hot and cold water faucets of utility sink. Instructed to install backflow prevention devices on faucets of utility sink and maintain.

- comments: observed no self closing device on washroom door. Instructed to install self closing device on washroom door and maintain.

- comments: observed lightshield in rear above utility sink stained with dead bugs on interior. Instructed to clean and maintain. Observed splashes on walls in hallway leading to rear storage. Instructed to clean and maintain. Observed white substance on floor, along bases of wall and in corners of rear storage closet used to store baking products. Instructed to clean and maintain.

Observed damaged tiles at base of front of utility sink area. Instructed to repair/replace damaged tiles at base of front of utility sink area.

- comments: observed dirt and dust buildup on floor vents underneath handwashing sink. Instructed to detail clean floor vents and maintain. Observed dust on ceiling vent cover in washroom. Instructed to clean and maintain.
