1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed person in charge without proof of food safety management training during time of inspection. Must provide.

2. City of chicago food service sanitation certificate
- comments: observed facility food handlers preparing and handling time and temperature control for safety(tcs) foods without original city of chicago food service manager and certificate on site. Informed person in charge city of chicago original food service certificate must remain posted on site and certified manager on site during tcs food prep, cooking, serving and operating hours. Priority foundation violation #7-38-012. Citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees in acknowledgment of reporting agreement. Priority foundation violation #7-38-010. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed hand sink in front food prep area not accessible. Pizza boxes in front of hand sink and basin was filled with big food container. Instructed that sink must be accessible and maintained at all times. The items were removed during the inspection to correct the violation. Priority foundation violation #7-38-030(c). Citation issued. - cos

38. Insects, rodents, & animals not present
- comments: observed pest activity with approx. 20 small flies noted during inspection. Small fly activity was noted near the dish machine, wall in the rear food prep area. Instructed to consult pest control as additional services are needed to minimize or eliminate activity prior to re-inspection. Priority foundation violation #7-38-020(a). Citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed box of food stored on the floor in the walk-in cooler. Instructed manager to properly store and protect food items from contamination while stored by elevating at least six inches from the floor on raised shelving units.

40. Personal cleanliness
- comments: observed food handlers prepping food without hair restraints. Instructed person in charge that all food handlers must wear a hair restraint.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed milk crates being used as shelving throughout the rear storage area near the three compartment sink. Must remove and provide shelving that is 6' off the floor and provides floor accessibility to ensure adequate facility cleaning.

- comments: observed 3-compartment sink stoppers missing/not working at left side compartment. Instructed manager must provide working stoppers at 3-compartment sink.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed facility without a chemical test kit for sanitizer or for ware washing area. Informed person in charge a test kit that accurately measures the concentration in mg/l of sanitizing solution must be provided. Priority foundation violation #7-38-005. Citation issued.

49. Non-food/food contact surfaces clean
- comments: observed the unused freezer stored in the rear food prep/dish area to have food splash and food debris. Instructed to clean and maintain until repaired and/or removed off premise.

50. Hot & cold water available; adequate pressure
- comments: observed no water at the two hand sinks in the front and rear food prep/dish area. Water is not sufficient capacity to meet the peak water demands. Informed person in charge facility must repair and maintain. Priority foundation violation #7-38-030(c). Citation issued.

52. Sewage & waste water properly disposed
- comments: observed inadequate waste and sewage water disposal sitting in the front food prep hand sink. Must remove water from hand sink and keep clean and maintain at all times. Priority violation #7-38-030(c). Citation issued.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle in the women toilet room. Instructed to provide and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed unused and soiled 2-door freezer stored in the rear food prep/ dish area. Instructed manager to remove all unused articles and equipment to prevent pest harborage.

57. All food employees have food handler training
- comments: observed no proof of training for all food handlers. Instructed to provide and maintain records at all times.

58. Allergen training as required
- comments: observed no certified food manager with allergen training. Instructed any persons with the city of chicago food sanitation certificate to comply with completion of training. Must provide documentation.
