2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager on the premises while tcs foods (coffee drinks with milk) were being prepared and served. Instructed management that a city of chicago certified food manager must be present at all times while tcs foods are being prepared or served. Priority foundation 7-38-012. Citation issued.

22. Proper cold holding temperatures
- comments: found cold tcs foods at improper cold holding temperatures inside the 3-door cooler. Milk, half & half, heavy whipping cream, cream cheese, ricotta cheese, cake mix with ricotta, eggs, and eggs whites found at temperatures ranging from 46f to 50f. Instructed to discard and denature said tcs foods and always maintain all cold-holding tcs foods at or below 41f. Priority violation 7-38-005. Citation issued. Food discarded.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed equipment not maintaining proper temperature. Noted temperature of the 3-door cooler in the rear prep area to be improper at 46. 0 f. Unit tagged and held for inspection by the chicago department of public health and may not be used until re-inspected by cdph and maintaining proper temperature of 41f and below. Priority violation 7-38-005. Citation issued.

36. Thermometers provided & accurate
- comments: observed no ambient air thermometers inside the 1-door dairy cooler underneath the front service counter and inside the 2-door household refrigerator. Instructed to provide conspicuous and easily readable thermometers inside all cold-holding units to monitor the ambient air temperature of equipment properly.

38. Insects, rodents, & animals not present
- comments: found incomplete pest control log book. Observed no recent inspection receipts (at least twice a year per code) on-site at the time of inspection; last report provided on site is dated 7/16/2022. Instructed to have a pest control log book on site all times with updated inspection reports. Priority foundation 7-38-020(c). Citation issued.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed an unsealed wood being used as shelving in the dry storage closet. Must seal or paint shelves so that they are smooth, non-absorbent, and easily cleanable and maintained.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed facility without a chemical test kit on site for the sanitizer (quat) being used for sanitizing at the three-compartment sink. An informed person in charge of a test kit that accurately measures the concentration in mg/l of a sanitizing solution must be provided. Priority foundation violation 7-38-005. Citation issued.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no self-closing device on the employee washroom door. Instructed management that all washroom doors must be completely enclosed and provided with tight fitting and self-closing doors.
