41. Wiping cloths: properly used & stored
- comments: 4-901. 12 observed soiled wiping cloths stored throughout the front prep area improperly on prep tables. Instructed to store inside sanitizing bucket with at least 50ppm solution

49. Non-food/food contact surfaces clean
- comments: 4-601. 11(c) observed excessive dried food splatter on icee machines in front prep, on exterior surfaces of hot hold units containing sauces (bbq, buffalo, etc. ), inside 2-door 7up cooling unit in rear prep, and interior surfaces of 2-door with cold hold attached. Instructed to remove build-up on said surfaces.

55. Physical facilities installed, maintained & clean
- comments: 6-501. 13 observed pooled chicken blood and water on the floor inside the walk-in cooler. Instructed to remove, clean and maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: 6-303. 11 observed burned-out light bulbs in the rear prep area. Instructed to replace and increase lighting

58. Allergen training as required
- comments: 2-102. 13 the city of chicago certified food manager without proof of allergen training. Instructed to provide and maintain on-site for review as required.
