37. Food properly labeled; original container
- comments: observed some bulk storage containers not labled with name of product inside of container; instructed to label all that are not labeled.

38. Insects, rodents, & animals not present
- comments: observed rodent activity on premises. Found 50 mouse droppings on the floor under storage shelves and under front counter. Instructed to clean all areas and have pest control company service prior to reinspection next week. Priority foundation violation 7-38-020(a) citation issued.

41. Wiping cloths: properly used & stored
- comments: wiping cloths not stored in a sanitizing solution as required; instructed to store wiping cloths in prep area in a sanitizing solution when not in use.

49. Non-food/food contact surfaces clean
- comments: instructed to clean and maintain the following: exterior surfaces of cooking equipment, bulk storage container exterior surfaces, interiors of refrigeration units where needed, bottoms of prep tables, exterior surfaces of garbage containers, can opener sleeve on prep table, exterior surfaces of rice cookers. Clean interior surface of ice machine.

55. Physical facilities installed, maintained & clean
- comments: instructed to clean the floor in areas where droppings observed and under cooking equipment. Instructed to clean ceiling vents where needed to remove dust observed.

58. Allergen training as required
- comments: instructed the food service manager to obtain the required food allergen training.
