16. Food-contact surfaces: cleaned & sanitized
- comments: 4-602. 11 observed the cutting board attached to the prep cooler with dark grooves. Instructed to remove grooves or replace cutting board.

37. Food properly labeled; original container
- comments: 3-302. 12 observed spices out of the original packaging and held in containers by the wok station without labels containing the product name. Instructed to provide common name labels for all food items that are out of the original packaging.

38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of pest activity on site. Observed about 10 rodent droppings in the toilet room around the hot water heater, about 10 rodent droppings behind the ice machine and about 5 rodent droppings under the 3 compartment sink. Instructed to eliminate rodent activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

43. In-use utensils: properly stored
- comments: 3-304. 12 observed plates are being used to scoop foods and plates being stored in the container of food. Instructed to use scoops and do not store scoops in product.

- comments: 4-502. 13 observed plastic grocery bags being used to store foods in the walk in freezer. Instructed to not use plastic grocery bags to store any foods.

55. Physical facilities installed, maintained & clean
- comments: 6-501. 12 observed accumulated grease debris on the floors under the wok station and under the fryers. Instructed to clean and sanitize.
